בן מלכה ויהודה לייב. נולד בשנת תרמ"א (בינואר 1881) להורים בעלי אחוזות בפלך מינסק שברוסיה. בן חמש עשרה נכנס ללמוד בישיבא טלז ושם נמנה עם חריפיה. אביו קיווה לראותו רב בישראל, אך בגיל שמונה עשרה עזב חיים את הישיבה ועבר למינסק כדי ללמוד שם בגימנסיה. לאחר שקיבל את תעודת הבגרות יצא ללמוד רפואה. ובשנת 1912 גמר לימודים אלה באוניברסיטה של ברלין. עם פרוץ המלחמה העולמית התגייס לצבא הרוסי כרופא ועשה שלוש שנים בחזית. לאחר המלחמה התיישב בעיר סרנה שבווהלין ושם עבד במקצועו וגם התמסר לפעילות ציונית. הוא התחבב על תושבי העיר בזכות מסירותו כרופא, מרצו כפעיל הציבור והליכותיו עם הבריות.
בשנת 1921 עלה ד"ר יזרעאלי עם אשתו ובנו בן השנה לארץ ישראל. זמן מה התעכב בפתח תקוה בבית הוריו שעלו לארץ לפניו ואחר-כך התחיל לעבוד כרופא ברמלה. שם פעל הרבה לקירוב התושבים היהודים המעטים אל הערבים ולהגדלת הישוב המקומי. בסוף שנת 1923 עבר לעזה ופעל הרבה בקרב תושביה הערבים, סייע לעדה היהודית הקטנה והשתתף בייסוד "הלוואה וחסכון", בעיר.
במאי 1924 עבר לגור עם משפחתו בבאר-טוביה ואף כאן היה עיקר פעולתו הרפואית לטובת ערביי הכפרים הסמוכים, שרחשו לו כבוד וחיבה. במושבה רכש שטח קרקע וזמנו הפנוי הקדיש למשק, טיפל ברפת, בגן ובחצר, הוציא את פרותיו למרעה בבוקר וקיבל את פניהן בערב עם שובן מן השדה. גם מצא פנאי לפעילות ציבורית והיה חבר קבוע בוועד המושבה.
במאורעות תרפ"ט התכוננו ערביי הסביבה להתנפל גם על באר טוביה. ד"ר יזרעאלי הוא הרוח החיה בהגנת המקום, עומד בראשה, מאמן בנשק את התושבים, ומעודדם ומחזקם.

He was born in 1881 to parents, Malka and Yehuda Leib, who owned estates in the Minsk region of Russia. At the age of eighteen he left the yeshiva and moved to Minsk to study at the Gymnasium. In 1912, Chaim completed his studies at the University of Berlin, where he joined the Russian army as a physician and spent three years at the front, where he settled in the city of Sarna in Volhynia where he worked in his profession and devoted himself to Zionist activities. His devotion as a doctor, his energy as a public activist and his conduct with people.

In 1921, Dr. Yizraeli immigrated to Israel with his wife and one-year-old son for a while in Petah Tikva, where he worked as a doctor in Ramle, where he worked to bring the few Jewish residents closer to the Arabs and to increase the local community.

At the end of 1923 he moved to Gaza and worked extensively among its Arab residents, helped the small Jewish community and participated in the establishment of a "loan and savings" in the city. On September 25, 1929, the attacks continued throughout the day and at night, and the defenders, again under his command, repulsed the attack. During the attack he was shot and killed on the spot. He was laid to rest in the cemetery of the Beer Tuvia settlement.

His name was commemorated in the Yizkor Book in 1929 and in the Jabotinsky Institute Yizkor Book.
