Is there a known Reb Noson's "family tree" which would reveal the surnames of his family history?
Does the Breslov Institution have his family tree?

Reb Noson had a son named Shachne. Shachne is a name from (at least)
Talmudic times (see Shabbat 12b, with Tosfot s.v. Shavne). Sachnuyt sounds
like a Slavic version of the name.
The following is from "Through Fire and Water", a biography of Reb Noson,
published by Breslov Research.
Reb Noson had seven children (the number is the place in birth order; I
believe I got it right!):
Chana Tzirel 4
R' Dovid Tzvi 3
Meir (died at the age of three)
R' Nachman Sternhartz 5
R' Shachneh 1
R' Yitzchok 2
R' Yosef Yonah 6
