Certificate of Incorporation
of the
FIRST BURSHIVKER SICK AND BENEVOLENT ASSOCIATION.
We, the undersigned, Elle Singer, Sam Shedelman, Jacob Spitzinosky, Aaron Malsky, Morris Orenstein, Morris Schwartz, David Davidson, Israel Friedman and Chiam Kelnitzky, do hereby certify as follows:-
FIRST:- That a notice of the meeting of the members of the First Burshivker Sick and Benevolent Association, to determine whether such corporation should be incorporated, was duly given in pursuance of Section 81, of the Religious corporation law, and such meeting was held in accordance therewith, on the 21st day of October, 1904.
SECOND:- Elle Singer was the presiding officer of such meeting, and Sam Shedelman, Jacob Spitzinosky, Aaron Malsky, Morris Orenstein, Morris Schwartz, Chiam Kalnitzsky, David Davidson and Israel Friedman were present and voted thereat.
THIRD:- That at said meeting it was determined to incorporate said corporation as a religious corporation in pursuance of Sections 82-83 of the Religious Corporation Law.
FOURTH:- The name of the proposed corporation decided upon at said meeting is "FIRST BURSHIVKER SICK AND BENEVOLENT ASSOCIATION".
FIFTH:- That the said corporation has no permanent place of worship, and at said meeting it was decided that it should not have any permanent place of worship.
SIXTH:- The number of trustees decided upon at said meeting was nine.
SEVENTH:- The names of the trustees elected by said meeting to hold office until the first annual election are respectively, Elle Singer, Sam Shedelman, Jacob Spitzinosky, Aaron Malsky, Morris Orenstein, Morris Schwartz, Chiam Kolnitsky, David Davidson and Israel Friedman.
Eighth:- The names of the trustees elected by such meeting to hold office until the second annual election are Elle Singer, Aaron Malsky, David Davidson and Morris Schwartz.
Ninth: The names of the trustees elected by said meeting to hold office until the third election, are respectively Chiam Kolnitzky, Morris Orenstein and Jacob Spitznosky.
Tenth: The date decided upon such meeting for holding the first annual election and meeting to elect trustees for such corporation was the first Thursday in the month of December One Thousand Nine Hundred and Four.
In witness whereof, we made, signed and acknowledged and filed this Certificate on the 5th day of November, 1904.
